ISHRS MEMBERSHIP APPLICATION

Associate Member Application

Per ISHRS Bylaws, Associate Membership is limited to physicians who have an MD (Doctor of Medicine), DO (Doctor of Osteopathic Medicine), or non-U.S. equivalent medical degree.

Applicants for Associate Membership must complete the application and submit it along with the following: 

  • Current medical license: The medical license must have an issue date and an expiration date visible on it, and a current license in the area where the applicant practices. 
  • Current Curriculum Vitae (CV).

Application Fee: $150

Annual dues fees (subject to change and invoiced after acceptance).  $750
Mid-year dues fees are prorated accordingly for those approved in April.

New Full Member Application

This membership type is for individuals who are not currently members of the ISHRS.

Per ISHRS Bylaws, Membership is limited to physicians who have an MD (Doctor of Medicine), DO (Doctor of Osteopathic Medicine), or non-US equivalent medical degree, and a current license in the area where the applicant practices.

Applicants for Full Membership must complete the application and submit it along with the following: 

  • Current medical license for the jurisdiction in which the applicant will be listed on the ISHRS website. The medical license must have an issue date and an expiration date visible on it. 
  • One letter of recommendation is required from an ISHRS full or Fellow Member. The letter must be submitted on the letterhead of the recommending physician and signed.
  • Copy of current Curriculum Vitae (CV).
  • Completion of Minimum Educational Requirements.

To meet the Minimum Educational Requirement, applicants must earn 5 points from the Points Menu of which 3 points must be acquired from attending an in-person ISHRS World Congress. See the ISHRS website for the Points Menu with the list of approved meetings.

Alternatively, to elevate from Resident Member or Associate Member to full Member, one may complete an ISHRS Fellowship Training Program. To maintain Member status, the Member must earn 3 points every 3 years from the Points Menu, and there must be at least 3 points acquired from attending an in-person ISHRS World Congress every 6 years. Failure to meet the required points will result in the automatic change of membership status to Associate Member.

Adjunct Member Application

Applicants for Adjunct Membership must complete the application and submit it along with the following:

  • A letter of attestation of good moral character from a Fellow Member, Member, or Associate Member of the ISHRS. The letter must be submitted on the letterhead of the recommending physician and signed. 
  • Evidence of a doctorate in a biomedical-related area from their degree-granting institution.
  • A detailed statement outlining current academic activities in hair research.
  • Current Curriculum Vitae (CV).

Application Fee: $150

Resident Member Application

Applicants for Resident Membership must complete the application and submit it along with the following:

  • A letter from the Chief of Service or Program Director attesting to their enrollment in the residency program, the residents' start and end dates in the program, and recommending them for Resident Membership status in the Society.
  • Current Curriculum Vitae (CV).
  • Current medical license.

Application Fee: $75

Annual dues fees (subject to change and invoiced after acceptance).  $335
Mid-year dues fees are prorated accordingly for those approved in April.

Surgical Assistant Application

Applicants for Surgical Assistant Member category must complete the application and submit it along with the following:

  • Letter from the employing ISHRS physician member (Fellow Member, Member, or Associate Member) who has a clinical hair restoration practice and performs hair restoration surgery, attesting to the fact that the applicant is a current employee of that member and assists during surgery.
  • Copy of current healthcare license. The license must be in the same jurisdiction in which the applicant is located, and the jurisdiction as the physician member who must be licensed in that state/location. 

Application Fee: $75

Annual dues fees (subject to change and invoiced after acceptance).  $275
Mid-year dues fees are prorated accordingly for those approved in April.
